OPBEA Get Involved | Tribute Cards

The Oak Park Business and Education Alliance has a great way for you to honor a special event in someone’s life or send a condolence in memory of someone.

We have designed a Tribute card that can be mailed out to acknowledge a donation made to the Oak Park Business and Education Alliance.

It’s easy! All you have to do is make a donation either online with a charge card or mail a check to our office, along with e-mailing or mailing the following information:

(Please print or type all information clearly)

Name and address of person/persons receiving the tribute.
Person being honored or in memory of whom.
Name and address of person/persons donation is from.
Phone number of a contact person sending donation.
